The PROLENSA Part D Program offers savings for Medicare Part D patients. Most eligible patients pay no more than $60 co-pay on their PROLENSA® prescription.

ACTIVATE:

Patients can activate the print coupon by calling 1-866-686-0091. To activate the online download coupon, patients must agree to the statements on www.prolensapartdcoupon.com.

REDEEM:

Patients can redeem the coupon card at a participating retail pharmacy with their prescription and insurance cards.

MAIL:

Patients sign and mail the prewritten letter to their health plan. Health care plan address can be found on their Medicare provider card.
