The PROLENSA Part D Program offers savings for Medicare Part D patients. Most eligible patients pay no more than $60 co-pay on their PROLENSA® prescription†.
Patients can activate the print coupon by calling 1-866-686-0091. To activate the online download coupon, patients must agree to the statements on www.prolensapartdcoupon.com.
Patients can redeem the coupon card at a participating retail pharmacy with their prescription and insurance cards.
Patients sign and mail the prewritten letter to their health plan. Health care plan address can be found on their Medicare provider card.
